Be There San Diego has developed the Heart Attack and Stroke Free Zone project and the Southeastern San Diego Cardiac Disparities project.

Heart Attack & Stroke Free Zone Project

With funding from the Centers for Medicare and Medicaid Services, Be There San Diego is working with 9 health care teams across San Diego County to deploy a health coach program to support enrolled patients in treatment for cardiovascular risk factors. Participating health care teams include Arch Health Partners, Neighborhood Health Care, North Coast Family Medical Group, San Ysidro Health Center, Scripps Clinic, Scripps Coastal, Sharp Rees-Stealy, University of California San Diego Family Medicine Group and Vista Community Clinic.

Southeastern San Diego
Cardiac Disparities Project

With funding from the Centers for Disease Control, Be There San Diego has partnered with the Southeastern San Diego Community Advisory Committee to develop a heart health intiative in Southeastern San Diego.
By working with the faith-based organizations and primary care providers in that community we are positively impacting cardiovascular health disparities
